Are you a compassionate, experienced Registered Nurse ready to make a difference across the state of California? Join our team as a Traveling Hospice/Home Health Care Registered Nurse, where you’ll embark on meaningful multi-week assignments while working directly for our company.
What You’ll Do
Travel and Support: Embark on multi-week assignments (8-13 weeks) across California to provide care in locations with vacant positions, increased census, or additional staffing needs. Patient Care: Deliver exceptional patient care by admitting patients, performing emergency and routine assessments, and ensuring documentation directly at the bedside. Clinical Coordination: Collaborate with interdisciplinary teams (Physical Therapy, Occupational Therapy, Social Workers, Home Health Aides) to ensure seamless care aligned with the patient’s Plan of Care (POC). Symptom Management: Work with the Hospice Interdisciplinary Team (IDT), Medical Director, and Attending Physician to alleviate symptoms and prevent unwarranted hospitalizations. Preceptor Role: Act as a mentor to onboard and train new team members while promoting best practices. Case Management: Coordinate clinical and psychosocial care to meet patient and family needs, ensuring continuity and quality care. Education: Teach patients and caregivers about disease management, symptom control, wound care, and IV therapy to empower them with the knowledge needed for successful at-home care. Supervision: Oversee Licensed Practical Nurses (LPN) and Aides to ensure high-quality care delivery. Administrative Tasks: Complete referral and admission assessments, develop and update care plans, and maintain documentation according to policy standards. Perform additional duties as assigned by leadership.

What We’re Looking For
- Licensure: Must hold a current and valid Registered Nursing (RN) license for the state of California.
- Education: Associate or Bachelor’s degree in Nursing required.
- Experience: A minimum of 1 year of nursing practice preferred, with experience in Home Health, Hospice, Oncology, or Long-Term Care highly desirable.
- Skills: Strong coordination, communication, and interpersonal skills to work effectively with patients, families, and teams.
